Directions:
This evaluation is to be filled out by the
field supervisor of the student intern. It allows the supervisor to evaluate
the intern’s performance and communicate this evaluation to the student’s
professor. This evaluation will contribute toward the student’s final grade.
This evaluation should not take more than 10 minutes of your time and is a
requirement of the course. Your response to this performance evaluation is very
much appreciated. Please answer each item using the scale provided. There is
space at the end of this form for general comments. It is encouraged that you
